| Description of Each (Type, Location, Resolution): | 11/12/2025 Kitchen employee cut tip of finger with knife. Went to ER for treatment and returned to work. 11/28/2025 Employee slipped and fell in ballroom and struck right elbow on floor causing a stress fracture. Employee went to urgent care, saw an orthopedic and returned to work. Could fine no reason for slippery conditions on floor. Had flooring company out to inspect floor and rebuffed floor as a precaution |

| Summary of Issues Found and Actions Taken: | Missing blank on fuse block in lighting panel, replaced. Three batteries were bad on exit signs and all three signs were replaced. One battery on a set of emergency lights was bad and unit was replaced. Sensor for smoke detection on ballroom AC unit duct was faulty and was replaced. Items were blocking access to the main fire alarm panel and were removed. Area was flagged for no storage in the future. |
